How to proceed

Please fill out the form below to log your Hope Opportunities points. Make sure to include any proof you can to verify your activities.

When logging points:

* Points must be logged within 60 days of the activity accomplished/completed.
* Points will be recorded by Hillcrest Hope on the 1st and 15th of each month.
* Points can be redeemed once they have been recorded by Hillcrest Hope staff and you have received an e-mail confirming your point total.
* With any incentives that must be received in person (gas cards, pantry shopping, Thrift Store vouchers), your incentive will be provided to you within 3-5 business days.
